Enrollment Process for Parents/Guardians who want to complete their enrollment process online

  • STEP 1 – GATHER AND ELECTRONICALLY SAVE THE MANDATORY DOCUMENTS NEEDED FOR ENROLLMENT.

    These documents must be downloaded during the pre-enrollment process.

      • Parent/Guardian’s Picture ID
      • Proof of Residency (utility bill or lease agreement dated within 60-days)
      • Proof of Child’s Birth Date
      • Child’s Immunization Record
      • Child’s Physical Form or Proof of Upcoming Physical Appointment. Only if one of the following applies to your student.
        • Students enrolling into grades Transitional Kindergarten (TK), Kindergarten and 1st Grade.
        • Students entering FUSD from another country must have a completed physical and TB test.

    Click on the link below for a list of acceptable documents according to the grade you are enrolling for.

    The following documents are not mandatory, however, if they apply to your student, please, download the most updated copy of the document. Some of them will assist us in placing your student in the right program and/or school.

      • Special Education Documents – Copy of most recent IEP
      • Foster or Legal Documents – Foster placement, Court documents, and/or Restraining Orders
      • Approved Intra OR Inter District Transfer Letters
      • Previous School Transcripts OR Check-Out Grades (High School Students only)

    Some enrollments may require additional enrollment forms to be completed for an enrollment.  These forms are located under our Additional Forms Tab.

    • An adult family member who is not biological or legal guardian will need a Caregiver Form.
    • A family not able to show proof of residency will need to fill out a Declaration of Residency Form.
    • A student needing a physical we have a physical form you can print out to take to your child’s doctor.


    STEP 2 – FILL OUT THE ENROLLMENT FORMS ELECTRONICALLY.

                    Click on the link below to complete and save the enrollment forms.


    STEP 3 – PREENROLL YOUR STUDENT.

    Click the orange “Pre-enroll” button below to begin the enrollment process.  You will be asked to upload your mandatory enrollment documents and the completed enrollment forms.

    Your documents and forms will be verified and processed by one of our enrollment clerks within a few days. Once this has been completed the clerk will contact the parent/guardian for them to come to the Enrollment Center.  The Enrollment Center is located at 9548 Citrus Ave., Bldg. A., Fontana Ca 92335.

    Parent/Guardian will pick-up the student’s completed enrollment packet at the receptionist desk. They will also pick-up the student’s laptop at our Laptop Distribution and Parent Tech Support Center (located in the Enrollment Center Building).

    Once the parent/guardian pick-up the school enrollment packet and the laptop, we ask that the parent/guardian take the school enrollment packet to the school.  This lets the school know of their new student and it gives them the opportunity to assign the student to a teacher and classroom or help with creating a schedule for our middle and high school students.
