Pre-Enroll Process for New or Returning Students

  • Enrollment Process for Parents/Guardians who want to pre-enroll and make an appointment or come in as a “walk-in” to enroll.

    STEP 1 – GATHER THE MANDATORY DOCUMENTS NEEDED FOR ENROLLMENT.

    These documents must be presented at the time of the enrollment.

      • Parent/Guardian’s Picture ID
      • Proof of Residency (utility bill or lease agreement dated within 60-days)
      • Proof of Child’s Birth Date
      • Child’s Immunization Record
      • Child’s Physical Form or Proof of Upcoming Physical Appointment. Only if one of the following applies to your student.
        • Students enrolling into grades Transitional Kindergarten (TK), Kindergarten and 1st Grade.
        • Students entering FUSD from another country must have a completed physical and TB test.

    The following documents are not mandatory, however, if they apply to your student, please, bring in the most updated copy of the document. Some of them will assist us in placing your student in the right program and/or school.

      • Special Education Documents – Copy of most recent IEP
      • Foster or Legal Documents – Foster placement, Court documents, and/or Restraining Orders
      • Approved Intra OR Inter District Transfer Letters
      • Previous School Transcripts OR Check-Out Grades (High School Students only)

                    Some enrollments may require additional enrollment forms to be completed for an enrollment.  These forms are located under our Additional Forms Tab.

    • An adult family member who is not biological or legal guardian will need a Caregiver Form.
    • A family not able to show proof of residency will need to fill out a Declaration of Residency Form.
    • A student needing a physical we have a physical form you can print out to take to your child’s doctor.


    STEP 2 – PICK-UP OR PRINT-OUT THE ENROLLMENT FORMS.

    Enrollment Forms are available at the Enrollment Center (Bldg. A) in the ÂãÁÄÖ±²¥ Complex, or you may fill out and print our fillable forms below by selecting the correct link and bring them to your enrollment appointment.

    For students who have never been enrolled with FUSD, fill out these forms:

    STEP 3 – PREENROLL YOUR STUDENT AND MAKE AN APPOINTMENT OR WALK-IN

    FOR NEW FUSD STUDENTS (Students who have never been enrolled with FUSD): Click the orange “Pre-enroll” button below to begin the enrollment process.  After you pre-enroll your student, you will then be able to make an appointment to complete the enrollment process or you may come into the Enrollment Center as a walk-in. Walk-in times vary and are determined by the number of walk-ins and staff members on duty.

    FOR RETURNING FUSD STUDENTS (Students who have already attended a school in FUSD): You do not need to pre-enroll due to our system retaining your student’s basic information.  Please, click the link below to automatically take you to our appointment system to schedule an appointment that works best with your schedule, or you may come into the Enrollment Center as a walk-in. Walk-in times vary and are determined by the number of walk-ins and staff members on duty.

    STEP 4 – COMPLETE ENROLLMENT AT ENROLLMENT CENTER.

    Enrollments are processed at the Enrollment Center located at 9548 Citrus Ave., Bldg. A., Fontana Ca 92335.  We ask that you show up prepared with all printed mandatory documents and sign-in at our reception desk at least 10 minutes early for your appointment.

    Your appointment may be rescheduled if not all mandatory documents are present at time of enrollment.
