FAQ for Athletic Clearance.

What do you need to participate in Athletics?
All students must have the two following items completed in order to be "cleared" to participate on any athletic team, here at Jurupa Hills.
(1) ATHLETIC CLEARANCE ACCOUNT
All students must have an online athletic account completed. Log onto and register for an account.
(2) SPORTS PHYSICAL
Physicals must be completed after May 1st of the current school year. It must be signed and stamped by your doctor. You can turn it in by uploading it to your online account, or walking it in.

What can get in the way of a student being eligible to participate in ATHLETICS?
NO MEDICAL INSURANCE. ALL STUDENTS MUST HAVE MEDICAL INSURANCE. NO EXCUSES. IF A STUDENT DOES NOT HAVE MEDICAL INSURANCE, PARENTS CAN PICK UP A BROCHURE TO PURCHASE INSURANCE WITH THE MYERS AND STEVENS MEDICAL INSURANCE COMPANY. PLEASE SEE THE ATHLETIC SECRETARY FOR MORE INFORMATION OR .
NO CIF ACKNOWLEDGMENT FORMS. ONCE YOUR STUDENT HAS MADE THE TEAM ROSTER, OUR ATHLETIC TRAINER WILL MEET WITH YOUR STUDENT'S TEAM TO COMPLETE THE CIF REQUIRED PAPERWORK. THERE ARE A FEW FORMS YOUR STUDENT WILL TAKE HOME TO GET SIGNED. THESE FORMS NEED TO GET BACK TO THE ATHLETIC TRAINER ASAP. YOU CAN SEE THE ATHLETIC TRAINER FOR MORE INFORMATION.

Where can I get a Sports Physical?
1) J Hills will hold On-Site physicals about three times a year. The cost will be $20, cash only. Dates will be announced. Typically, dates are scheduled for May and August.

How do I get in touch with a coach or an Athletic Office Personnel?
You can always walk in to the office during regular business hours to speak with the Athletic Trainer and/or Secretary. Email is always an option and possibly the quickest response.
